Effective February 21, 2022, no PA is needed for insulin used during pregnancy. fb88Care will reimburse for Humulin and Novolin Neutral Protamine Hagedorn (NPH) insulin for pregnant patients without a PA when pregnancy is indicated on the prescription. If you receive a denial of a prescription for Humulin or Novolin NPH insulin for prior authorization and pregnancy is indicated on the prescription, please contact the Change Healthcare Pharmacy Helpdesk at 1-888-420-9711 for a pregnancy override.

Effective February 21, 2022, no PA is needed for insulin used during pregnancy.
