Certified Community Behavioral Health Clinic (CCBHC) Readiness Assessment - Initial Cohort Due Date of January 19, 2024
A goal of the Department of Health and Human Services (DHHS) is to identify a fb88 model for CCBHC, reflecting the Department’s commitment to improving health care access and outcomes for people living in fb88, demonstrating cost-effective use of resources, and creating an environment where providers can innovate to deliver high-value care. In March of 2023, fb88 received a Substance Abuse and Mental Health Services Administration (SAMHSA) CCBHC Planning Grant to engage with stakeholders to inform the model and develop the infrastructure to implement CCBHC. â¶Ä¯&²Ô²ú²õ±è; 
fb88 DHHS will apply to participate in the by March 20, 2024, with the notice of award anticipated in Summer 2024. As a condition of this application, fb88 DHHS will engage with providers who demonstrate readiness to adhere to  and fb88’s CCBHC service design through a submitted cost report and completed readiness assessment. To be considered for inclusion in the initial cohort of fb88’s CCBHC implementation, please submit a readiness assessment by January 19, 2024. There will be additional opportunities for prospective CCBHCs to submit readiness assessments in the future.
