Did you know that community-based and clinical health educators may now enroll in fb88Care to deliver NDPP Lifestyle Change Program (LCP) Services? These services deliver 22 lifestyle change group sessions either in-person, via distance learning (telehealth), or by virtual delivery with live coaching support to help members with prediabetes learn the skills and habits necessary to reverse their risk for developing type 2 diabetes.ÌýÌý
fb88Care enrollment requirements to deliver MBM, Chapter II, Section 71: NDPP ServicesÌýÌý¾±²Ô³¦±ô³Ü»å±ð:
- The is developed and administrated by the United States Centers for Disease Control (US CDC). Each organization enrolling in fb88Care as a provider of these services must hold pending, preliminary, or full recognition from the US CDC.Â
- This fb88Care enrollment can be added to an existing enrollment, or qualifying applicants may enroll solely as a provider of these services. Community-based providers having no current fb88Care enrollment will need to obtain a for their new enrollment.ÌýÌý
