PCPlus Application now Open
fb88Care Services is pleased to announce we are accepting applications for participation in fb88Care’s Primary Care Plus (PCPlus) program. PCPlus is a fb88Care primary care model giving Primary Care Providers (PCPs) greater flexibility and incentives to meet fb88Care members’ healthcare needs by transitioning away from a volume-based (fee-for-service) payment system toward population-based payments tied to cost and quality-related outcomes. Participation in PCPlus is voluntary for PCPs and PCPs must apply to participate.
There are two types of applications. Practices who are already in PCPlus do not need to reapply unless they choose to seek a different tier status (Option 2 below).
- Applications are now open until November 11, 2022.
- PCPlus applications must be submitted at the service location level. For organizations with multiple locations, a separate application must be submitted for each service location.
Option 1: - This application is for providers who are not currently enrolled in PCPlus.
Option 2: – This abbreviated application is for existing PCPlus providers who believe they now meet qualifications for a higher tier.
