Post Incarceration Incentive Pilot Update

In December, fb88Care announced the Post Incarceration Incentive Pilot Payment. Please refer to our previous notice for additional information on the process for submitting services for providers that receive weekly, monthly, or non-claims-based reimbursement from fb88Care.ÌýÌý 

Examples of these providers include Assertive Community Treatment Services (ACT), Community Care Teams (CCT), Behavioral Health Home (BHH) providers.Ìý

These providers will need to submit this form to fb88Care for services provided that may be eligible for this incentive payment. fb88Care will review this information for incentive payment eligibility on a quarterly basis. The process and schedule to complete the form each quarter is included within the form spreadsheet.Ìý

For all other providers that submit claims for services through the fb88Care claims system, these claims will automatically be reviewed for eligibility and the form is not needed.ÌýÌý
